Consultation form
The embeddable call-back form that turns website visitors into booked consultations.
The consultation form lets a visitor to a clinic’s website ask for a call back. Voxlera’s AI assistant then calls them in the time of day they chose, finds a free consultation slot with one of the clinic’s doctors, and books it.

What the visitor fills in

The form also tells the visitor, before they submit, that the call is placed by an automated assistant and is recorded. If the clinic has a privacy notice on file, a link to it appears under the consent box.
Language
The form appears in the clinic’s default language (English, Turkish or Russian; other clinic languages show the English form). It does not follow the visitor’s browser language, so a Turkish clinic’s form is always in Turkish.
What happens after a submission
Voxlera records the request and the visitor’s consent to be called and to receive a text message about it.
The first call is scheduled inside the chosen band, using the clinic’s local time:

The AI assistant introduces itself as automated, confirms the person is still interested, and offers the earliest free consultation slots across the doctors the clinic has enabled for consultations.
When the person picks a slot, it is booked in that doctor’s calendar and they get a confirmation by text message.
If nobody answers, Voxlera tries again: up to 3 attempts, at least an hour apart, each inside the chosen band. A request that hasn’t been booked within 48 hours expires.
If the same person submits the form again while their first request is still open, Voxlera updates the open request instead of calling them twice.
Note
Clinics also control the hours in which their AI agents are allowed to call. If those hours end before 20:00, an evening request may be called the next morning. See Consultation requests in the clinic guide.
Before you go live
Make sure the clinic has:
- at least one doctor with a connected calendar and consultations switched on (otherwise the assistant has no slots to offer),
- Automatic Outreach switched on in the dashboard, because consultation calls are placed automatically like the clinic’s other agent calls, and
- consultation calls enabled for the clinic (their Voxlera onboarding contact can confirm this).
Then make a test submission with a phone number you can answer.
